Purpose and scope

Supports Pharmacy Strategy Team in identifying business partnerships/opportunities within the community that can be capitalized for Advanced Care Specialty Pharmacy (ACSP) network with positive returns. Promotes and enhances market presence, and brand visibility of our ACSP network. Offers innovative leadership in identifying the untapped market. Additional responsibilities would include trouble shooting, issues resolution and management of special projects.


  • Conducts outreach activities within the community to help identify potential Hep C patients who should be screened and treated appropriately

  • Enhances public awareness regarding Hepatitis and support services available

  • Collaborates with local GPs to help create a mechanism that could expedite the screening process for Hep C patients

  • Participates in relevant professional education events and works closely with patient advocate groups

  • Works closely with key opinion leaders within the field of hepatology to identify and address any system gaps that may have hindered patients access to Hep C screening and treatment

  • Works in collaboration with Health Units, Outreach clinics, HIV and mental health care providers, addiction teams

  • Delivers nursing care in accordance with all required internal and external professional practice standards

  • Coordination services: case management, facilitation for timely access to the respective Patient Support Programs, consultation support, administrative paperwork as required

  • Works in collaboration with Patient Support programs and follows up with approvals

  • Educates patients on liver health, treatment/s prescribed and follow-up care

  • Educates on follow-up care if treatment is not prescribed

  • Reduces barriers to access to care with flexibility and accessibility

  • Works as a professional team member

  • Identifies key areas of territory-focus based on business opportunities and return on investment

  • Promotes the key differentiators and capabilities of ACSP network to key opinion leaders identified for the targeted specialty field(s)

  • Supports assigned high value accounts to ensure successful prescription sales to ACSP network

  • Leads, organizes and executes outreach activities to help increase ACSP network visibility, awareness and business sales

  • Leadership role in Outreach activities to optimize linkage to care and treatment

  • Rolls out marketing programs and initiatives in field

  • Develops relationships with key national and community associations related to the targeted specialty field(s), to help advance market reputation of our pharmacy brand and to drive prescription sales

  • Applies clinical expertise, nursing judgement and established medical criteria to provide case management insights to key stakeholders in relation to specific clinical programs.

  • Offers leadership, clinical training and operational support in house as well as targeted clinical sites

  • Assists in staff development

  • Manages and submits all required reports, budgets, territory business plans and other territory deliverables measured against KPIs and territory goals and responsibilities

  • Other Community Outreach Specialist related tasks as assigned by the Clinical Sales Specialist and/or the Director of Pharmacy Strategy


  • Registered Nurse, with a valid certificate of registration from the provincial licensing body

  • 3 or more years of nursing experience (as a Hepatology Nurse)

  • 3 or more years of fibroscan screening and analyzing experience

  • Strong decision making skills

  • Previous experience with project and case management

  • Excellent oral and written communications skills

  • Strong customer service orientation

  • Prior experience working with patients, health care providers and other stakeholders

  • Excellent organizational skills with good attention to detail

  • Prior experience in managing multiple projects simultaneously

  • Prior experience in using Microsoft Office Products and Web based applications

  • Has the use of an insured motor vehicle for work-related travel and a valid driver’s permit

Experience and required skills

  • Three (3) years of related experience

  • Bilingual (French/English) preferred

  • Demonstrated knowledge of medical criteria

  • Effective organization, coordination, and communication skills.

  • Professional and personal effectiveness in establishing relationships with clients, customers, peers, and staff.

  • Ability to work as a team member coordinating client care

  • Computer literacy
