The Patient Education Coordinator is an important role in our team. The nurse is passionate and is committed to provide a high level of patient care and counseling patients enrolled in the Medication Therapy Program. The ideal candidate has great communication skills, problem solving skills, and must be self-motivated. This person will be driven to help team members to achieve the mission of providing high level of care to patients.
Essential Duties and Responsibilities
To counsel and educate new/existing patients on therapy so that patients receive complete information regarding their care
Follow checklist provided by client while providing counseling so that proper information is collected and the Business Intelligence group can produce accurate reports
Discuss potential side effects with patients to ensure they are educated about their treatment to maintain compliance
Identify and report adverse events (AEs) and product complaints (PCs) to the manufacturer, FDA, and/or pharmacists and pharmacy manager
Contact Physician’s office regarding questions, concerns or problems with the prescription before processing the order to other groups so that errors or discrepancies are corrected early in the workflow, making the process more efficient for other groups
Through patient follow-up conversations, communicate changes in therapy noted by patient so that new documentation from providers can be obtained by the Patient Support group
Ensure patient profile information is correct, especially address so that distribution can successfully mail the package
Transferring calls to the Patient Support group in the event a patient is due for a refill in order to eliminate team members having to place additional phone calls to patients
Arrange shipment for medication
5 years of relevant work experience; Nursing preferred
Registered Nurse with current unrestricted state licensure
Computer skills, including e-mail, word processing
Ability to communicate effectively orally and in writing
Ability to read, analyze and interpret practice standards, professional journals, technical procedures and government standards and regulations
Oncology experience preferred
Experience with MS Excel spreadsheet preferred
Attention to detail
Call center experience preferred
Additional Knowledge & Skills
A seasoned, experienced professional with a full understanding of area of specialization; resolves a wide range of issues in creative ways. This job is the fully qualified, career-oriented, journey-level position.
Associates or Bachelor’s degree in Nursing
RN with Current unrestricted state licensure
